Wraparound Facilitator

EmployIndy has partnered with an Adult & Child Health center focused on delivering evidence-based practices to clients of all ages who experience serious mental illness. Their community-based mental health programs are delivered by multi-disciplinary treatment teams who are specifically trained to offer the very best person-centered recovery supports in the behavioral health industry.

The Wrap Facilitator works in partnership with seriously emotionally disabled children, their families, and referral sources to facilitate high fidelity wrap services, with the goals of the child maintaining safely in the home, school and community. The Wrap Facilitator utilizes a holistic, strengths based, and family driven treatment approach to facilitate collaboration between the youth/family and all parties involved in the client's treatment, to ensure the youth/family's success and longer-term stability.
Wrap Facilitation is a dynamic position that encourages the team to utilize out-of-the box thinking in developing creative solutions. The wraparound model supports long-term stability for youth and families through a strengths-based, family-driven, team-supported, and culturally competent process. The holistic wrap model focuses on identifying the underlying needs of a youth/family through strengths-based discovery and is often successful for families who have had limited success with traditional outpatient treatment.

• Experience with Systems of Care preferred
• Available to work weekdays and evenings; Not "on-call" after hours
• Ability to maintain professional relationships with a variety of individuals
• Comfortable working in a range of settings, including: homes, the community, and schools
• Strong organizational skills
• Must hold a valid driver's license with acceptable driving record and provide own, insured, transportation
• Bachelor's degree in Social Work, Psychology, Human Services, or another related field
• Two years' experience with children ages 6-17 that have been identified as Seriously Emotionally Disturbed (SED)

• Case Management
• Care Coordination
• Facilitating the wrap treatment process for a caseload of intensive wrap families
• Advocate for family choice/voice in the wrap treatment process
• Ensuring clinical providers on each family's wrap team provide effective services to client/family
• Developing Care Plans and requesting additional service authorizations
• Completing regular assessments utilizing the CANS Assessment tool
• Develop comprehensive discharge plans

Qualifying experience INCLUDES experience working directly with youth with SED between the ages of 6–17 in a way that builds functional skills, such as group counseling, one-on-one counseling, provision of skills training, and/or provision of therapeutic recreational activities.

Also included would be:
• Experience providing therapeutic foster care or working in a capacity that may not involve mental health care, but where the work is targeted at a defined SED population.
• Experience in case management, therapy, and/or skills training in conjunction with a mental health center may also be considered as qualifying experience.

The SED experience requirement EXCLUDES incidental experience. This means that the work of the provider may have been with a youth with SED, but the defined work role was not intended to address the SED condition directly, so the experience does not qualify toward the requirement.

Examples of incidental experience include:
• An owner of a day care for children who throughout his or her years of experience has cared for children classified as seriously emotionally disturbed
• A bus driver with children on his or her bus route who have been classified as seriously emotionally disturbed
• The facilitator of a youth group or bible school whose groups included some children classified as seriously emotionally disturbed
